ما هو دواء Levothyroxine? 
دواء مشتق لتعويض الهرمون الدرقي.
ما هي الاستطبابات لاستخدامه؟
يستخدم في حالات قصور الغدة الدرقية ولتثبيط الهرمون المنبه للغدة الدرقية TSH, كما يستخدم في حالات الوذمة المخاطية الشديدة.
ما هي مضادات الاستطباب؟
1)فرط نشاط الدرق غير المعالج.
2)القصور الكظري غير المعالج.
3)احتشاء العضلة القلبية الحديث.
ما هي التأثيرات الجانبية لاستخدامه؟
العصبية,الاستثارة,الرجفان,ضعف عضلي,التشنج العضلي,التعرق,التوهج,عدم تحمل الحرارة,صداع,أرق,تسرع القلب,خفقان,خناق الصدر,فقدان الوزن الشديد,اضطراب الطمث,اسهال,اقياء,اختلاجات,اضطراب النظم,فشل القلب,سبات.
التداخلات الدوائية:
تنخفض الامتصاصية مع الحديد ,الكوليسترامين,كوليستيبول,مضادات الحموضة الحاوية على الالمنيوم والمغنزيوم,كربونات الكالسيوم,سيميتيكون,سكرالفات.قد يعدل الحاجة الى الادوية الخافضة للسكر.انخفاض فعالية العلاج المعوض للهرمونات الدرقية مع الاماتينيب.انخفاض التركيز البلاسمي للتريايودوتيرونين مع الاميودارون.انخفاض التركيز المصلي للتيروكسين مع الكاربامازبين,الفينيتوئين,الفينوباربيتال,الريفامبسين والاستروجينات.زيادة التأثير العلاجي والسام لليفوتيروكسين مع مضادات الاكتئاب ثلاثية الحلقة.قد يغير استجابة الوارفارين ومضادات التخثر الاخرى لنقص بروثرومبين الدم وقد تؤدي الى الوفاة.زيادة احتمالية ارتفاع التوتر الشرياني وتسرع القلب مع الكيتامين.
التحذيرات:
خناق الصدر,قصور القلب,السكري,البيلة التفهة,المسنون,قصور الدرق ,قصور الكظر,الوذمة المخاطية,لا يستخدم لمعالجة البدانة او لتخفيف الوزن.
What is Levothyroxine?
A derivative drug to replace the thyroid hormone.
What are the indications for its use?
It is used in cases of hypothyroidism and to inhibit the thyroid stimulating hormone (TSH). It is also used in cases of severe myxedema.
What are the contraindications?
1) Untreated hyperthyroidism.
2) Untreated adrenal insufficiency.
3) Recent myocardial infarction.
What are the side effects of using it?
Nervousness, agitation, tremor, muscle weakness, muscle spasm, sweating, flushing, heat intolerance, headache, insomnia, tachycardia, palpitations, angina, severe weight loss, menstrual disorder, diarrhea, vomiting, seizures, arrhythmias, heart failure  ,coma.
Drug interactions:
Absorption decreases with iron, cholestyramine, colestipol, antacids containing aluminum and magnesium, calcium carbonate, simethicone, sucralfate. May modify the need for hypoglycemic drugs. Decreased effectiveness of thyroid hormone replacement therapy with imatinib. Decreased plasma concentration of triiodothyronine with amiodarone. Decreased serum concentration  For thyroxine with carbamazepine, phenytoin, phenobarbital, rifampicin and estrogens. Increased therapeutic and toxic effect of levothyroxine with tricyclic antidepressants. May alter the response of warfarin and other anticoagulants to hypoprothrombinemia and may lead to death. Increased likelihood of arterial hypertension and tachycardia with ketamine.
Warnings:
Angina, heart failure, diabetes, trituria, the elderly, hypothyroidism, adrenal insufficiency, myxedema, it is not used to treat obesity or lose weight.
